Alpha-2Macroglobulinisaserineproteasesinhibitor,whichinhibitscoagulationbyinactivatingthrombinandKallikrein,itinhibitsfibrinolysisbyinactivatingplasminandinvolvedintransport.Alpha-2-Macroglobulinisalargeplasmaprotein,whichisproducedbytheliver,it’scomposedof4identicalsubunitsboundtogetherby-S-S-bonds.A2Misabletoinactivatemanykindsofproteinases(includingserine-,cysteine-,aspartic-andmetalloproteinases).A2Mhasa35aminoacid"bait"regioninitsstructure.Proteinasesbindandcleavethe“bait”regionbecomeboundtoA2M.Macrophagereceptorsrecognizetheproteinase-A2Mcomplexandclearitfromthesystem.A2MbindstoandremovesMMP-2andMMP-9(activeformsofthegelatinase)fromthecirculationusingscavengerreceptorsonthephagocytes.ThelevelsofAlpha-2-macroglobulinareincreasedinnephroticsyndromewhichisaconditionwherethekidneysstarttoleakoutsomeofthesmallerbloodproteins.Duetoitslargesize,A2-macroglobulinisretainedinthebloodstream.IncreasedproductionofallproteinscausesA2-macroglobulinconcentrationtoincrease.Chronicrenalfailuremightleadtoamyloidbyalpha-2-macroglobulin.A2Misraisedincirrhosis,pregnancyanddiabetes.
